Patient Centered Medical Home
All of our primary care practices are recognized as Level 3 patient-centered medical homes, the highest level of distinction from the National Committee for Quality Assurance. This means that our nurse care coordinators work with you and your healthcare team to manage your health and make sure you have the care and support you need to meet your health goals.
We offer two types of primary care: adult and family. Adult primary care providers specialize in treating adults age 18 and older, while family care providers focus on the health needs of every member of the family, from birth to geriatrics.
